Abstract
A 45-year-old man presented with an asymptomatic, red-purple, smooth surfaced nodule on the left knee 3 cm in size, which was present for several years. Elsewhere the diagnosis pseudo B-cell lymphoma was made. Physical examination revealed a similar lesion on the right buttock upon which an additional immunohistochemical staining was requested. This staining showed a predominance of lambda chains over kappa chains in the infltrate. Revision of the biopsy by the Dutch Cutaneous Lymphoma Working Group resulted in the diagnosis marginal B-cell lymphoma, probably primary cutaneous. This case report illustrates that the differentiation between a primary cutaneous B-cellymphoma (type marginal zone B-cell lymphoma) and a pseudo B-cell lymphoma can be diffcult. The literature is reviewed.
