Abstract
At recovery from anaesthesia for mediastinoscopy, relaxation appeared still to be present in a 48-year-old male. The patient had respiratory failure and was reintubated, after antagonising with neostigmine having no effect. On the intensive care unit weaning from mechanical ventilation was unsuccessful. An electromyogram showed Lambert-Eaton myasthenic syndrome, an autoimmune disorder of the neuromuscular cleft resulting in muscle weakness. Soon after initiating treatment with 3,4-diaminopyridine the patient could be weaned successfully from the respirator.
