Abstract
A 28 year old woman presented to the emergency department because of a suicide attempt with two teaspoons of powdered nutmeg three hours earlier. She presented with mydriatic pupils, dry tongue, epigastric pain, and described having dreaming sensations. On examination she was haemodynamically stable, tachycardic, apathetic and had an EMV score of 15. Standard laboratory tests gave normal values. The ECG showed a sinus tachycardia. The patient was observed until eight hours after ingestion, during which period all symptoms resolved. The most important clinical symptoms of a nutmeg intoxication are anticholinergic effects. Toxic effects can be expected from doses of 5-10 g. Symptoms are usually mild, beginning approximately 3-8 hours after ingestion and resolving within 48 hours. Death resulting from an intoxication solely related to nutmeg is exceptional. Management is generally supportive.
