Abstract
The Cedell fracture, an avulsion fracture of the posteromedial side of the talus is rare and easily missed, and could be diagnosed as a simple medial ankle sprain. Conventional radiology can be misleading. The fracture can be missed or assessed as an os trigonum. A CT scan is the appropriate diagnostic tool. Open reduction with internal fixation of the bone fragment to the talus is the optimal treatment. In a longstanding unrecognized fracture, excision of the bone fragment with the callus gives the best functional outcome.
